Psychodynamic Assessment of Adolescents based on Information from Patients and Their Parents

 In the assessment of adolescent patients with different levels of neurotic behavior (school refusal, eating disorder, obsessive-compulsive disorder etc.), making a psychiatric diagnosis is not sufficient. It is also important to conduct assessments based on psychoanalytic developmental psychology, with reference to adolescents' psychological development. Such assessments provide information as to how an adolescent has reached the impasse in the developmental tasks, and how to deal with them. The assessments are directly related to the psychological treatment. When conducting parents' interview, it is necessary to listen to not only the superficial problems of adolescents, but also the factors in their interpersonal relationships that trigger neurotic behavior. Therefore, such interviews should focus on the adolescent's problems and the parent-child relationships rather than dealing with the parents' personal affairs. However, it is important to evaluate the parents' marital relationship and its influence on the adolescent. Further, information about the bedroom- structure (who sleeps in which room) is of utmost importance. In conducting an adolescent's interview, he/she is encouraged to talk about oneself freely. It is important to listen about his/her interpersonal conflicts underlining the superficial symptoms or behavior, future hopes, revision of the ego- ideal, and heterosexual relationships. Furthermore, the extent to which the adolescent can reflect on oneself, or to which he/she acts out one's conflicts should be evaluated. Finally, the following points can be identified: what defense mechanisms are used, what the central anxiety is, and whether the adolescent has reached the developmental stage appropriate to his/her age or if he/she is still in a former developmental stage. Based on the information derived from the interviews of parents and adolescents, the appropriate psychotherapeutic strategy (psychoanalytic or supportive) and the need for parents' guidance can be determined.
