"Conditions Related to Sexual Health", a New Chapter in the ICD-11: Beyond the Dichotomy between "Non-organic" and "Organic" in Sexual Dysfunctions, and Toward Social Inclusion of Gender Diversity

 This report introduces a new chapter "Conditions Related to Sexual Health" in the eleventh revision of the International Classification of Diseases and Related Health Problems (ICD-11), which was released by the World Health Organization in 2018. In the previous version (ICD-10), "non-organic" sexual dysfunctions were classified in the chapter on Mental and Behavioral Disorders. On the other hand, many "organic" sexual dysfunctions were classified in the chapter on Diseases of the Genitourinary System, which hindered multidisciplinary approaches to problems related to sexual functions. ICD-11 overcomes the dichotomy between "non-organic" and "organic" in sexual dysfunctions and sexual pain disorders by means of classifying them in the new chapter, which is consistent with an evident interaction of physiological and psycho-social processes in sexual responses. Gender Identity Disorder in ICD-10 was renamed Gender Incongruence and was classified in this chapter in ICD-11 to separate transgender identity from the classification of mental disorders. This revision is intended to avoid stigma associated with the transgender identity and to facilitate public health services for transgender people, including those with non-binary gender identity. Lastly, the rationale for participation by psychiatrists in psychological assessment, diagnosis, and mental health support of Gender Incongruence, which is no longer a psychiatric disorder, was discussed.
