Two cases of primary hyperparathyroidism (PHPT) caused by adenoma of the parathyroid gland in elderly individuals are presented. Based on the patients' physical and neuropsychiatric symptoms, cognitive impairment, and postoperative courses, I discuss the pathophysiology and characteristic clinical symptoms of PHPT. The following results were obtained: 1) In our two cases of PHPT, the cognitive function, and physical and neuropsychiatric symptoms improved significantly after surgical resection of parathyroid adenoma. 2) Although the cognitive impairment caused by PHPT was similar to that observed in Alzheimer's disease, the impairment of attention was particularly marked. 3) The mechanisms of pain in biochemical PHPT should be further elucidated.
