End-of-life Care for Persons with Dementia

 The elderly population is increasing rapidly in Japan; therefore, the development of a support system for patients with dementia is an urgent task. In recent years, the palliative care approach, which has conventionally evolved according to the needs of patients with cancer, has been applied for persons with dementia. The palliative care approach aims to prevent and relieve discomfort by early identification, and detailed assessment and treatment of pain and other problems while respecting the dignity of the patient, and ensures open communication with honesty.
 Care for persons with dementia involves a number of key palliative interventions, including symptom management, especially pain control, which is insufficiently understood and undertreated, psychological support, decision making from diagnosis, and caregiver support. As palliative care is applicable in conjunction with other treatments, the palliative care approach is suitable for dementia care with comorbidity. Earlier advanced care planning for persons with dementia and their caregivers can help prepare for the complications of end-stage dementia.
