 In the trend of "From Institution-based Care to Community-based Care", various collaboration/cooperation approaches by team medicine have been conducted in the field of Mental Health Medical Welfare. Team medicine is defined as a group of professionals from different disciplines working in a coordinated fashion in order to improve the health of a patient. By understanding the advantages and disadvantages of collaboration with multiple professionals and their characteristics, we will be able to use team medicine to improve collaboration for supporting people living with diseases in the community. There are three core competencies in the collaborative capability, which are: the professional competence to be different from other professionals, the common competence such as the communication skills required by all professionals and the collaborative competence. In addition, in order for team members to successfully collaborate, a common language is required such as the International Classification of Functioning, Disability and Health (ICF). We integrate two models, the medical model and the social model, by using the ICF and understand 'difficulty in living' by positioning disability in the whole "living" of people. It is also important to constantly conduct evaluation checks and correct trajectories.
