Although the target for treatment of depression is recovery: maintenance of remission and improvement of social function, it has been widely recognized that accomplishment of recovery is not easy. As the first step of treatment is an accurate diagnosis,we have to make a differential diagnosis of adjustment disorder and bipolar disorder from depression in order to put treatment into the right direction. In the acute treatment phase psychoeducation has an important role in deepening the patients’understanding of depression and treatment. In the maintenance treatment phase the rework program, a rehabilitation program of depression is effective for stabilization of day-night rhythm, improvement of work ability and achievement of social stress coping. It is necessary for psychiatrists to take comorbidity of narcissistic personality disorder and pervasive developmental disorder into treatment strategy.
