PDFTBI and Temporal Pole Lesion ― Hypothetical Cognitive Mechanism of Delusional Perception ―

 The Conceptual Framework of Psychotic Disorder Following Traumatic Brain Injury(PDFTBI)was presented and two cases of PDFTBI who showed paranoid-hallucinatory state of persecution following TBI. Common lesion of two cases was Temporal Pole. We examined in the two cases facial expression rating task and confirmed the quasi-same pattern of perceptual bias to negative direction. Temporal Pole is crucial portion of visual, auditory and olfactory perceptual convergent locus which should be transferred to amygdale and its lesion might evoke isolation or insufficient condition of amygdale function. Temporal Pole is also one of the probable regions related with theory of mind.Duration of negative bias of emotional perception and insufficient theory of mind function should be one probable cause of latent period(several years)between the onset of TBI and the showing paranoid-hallucinatory symptoms of persecution. It will be necessary to elucidate further exact conditions of delusional hallucination of persecution following TBI because emotional cognitive disturbances after TBI are relatively well documented and that not all patients show delusional perception.
